The Role
As the MDS Coordinator, you’ll be a vital part of our nursing leadership team—helping ensure that every resident receives appropriate, personalized care while also supporting accurate and timely reimbursement through the MDS process. You’ll collaborate with the interdisciplinary team, manage all things Medicare/Medicaid documentation, and make sure we’re always survey-ready and staying compliant.
Key Responsibilities
MDS Maestro:
Lead the charge in completing timely, accurate MDS 3.0 assessments to support quality care and appropriate reimbursement.
Compliance Rock Star:
Ensure we’re always aligned with CMS regulations, Medicaid/Medicare guidelines, and facility standards.
Assessment Pro:
Put your top-notch nursing assessment skills to work to help craft care plans that are as individualized as our residents.
Collaborative Leader:
Work closely with the interdisciplinary team to gather input and provide insight that supports residents' physical, emotional, and restorative needs.
Care Plan Contributor:
Coordinate resident care planning meetings and ensure all documentation supports individualized care needs.
Documentation Detective:
Keep meticulous records, monitor for accuracy, and make sure everything is buttoned up, every time.

Qualifications
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Illinois
1 year of experience as an MDS Nurse (required)
Solid knowledge of MDS 3.0 and the Medicare/Medicaid reimbursement process
Strong assessment, analytical, and communication skills
Experience working within an interdisciplinary team
Proficiency with EMR/EHR systems and a good grasp of nursing documentation best practices
