Responsibilities of the Assistant Director of Nursing (ADON) - Clinical Services Supervisor (CSS) :
- The Assistant Director of Nursing (ADON) – Clinical Services Supervisor (CSS) collaborates with the Director of Nursing (DON) - Director of Clinical Services (DCS)
- Ensure the plans of care are implemented in a timely and efficient manner
- Manage the clinical team, develop and facilitate programs, and provide education to staff
- Plan and evaluate nursing systems, rotations and methods

The MDS / CARE PLAN COORDINATOR assumes the primary responsibilities for the care planning process and MDS completion within this facility and acts as the facilitator at the conference meeting. The MDS should be a Registered Nurse licensed by the State of Illinois.
Duties and Responsibilities
1. Setting up and maintaining an organized system by which Care Plan / MDS review dates are determined and schedules.
2. Notifying each department as to conference date, time, and meeting location as well as a list of specific residents to be reviewed.
3. Maintaining a record of attendance at meetings by all staff members.
4. Calling the meeting to order and acting as “timekeeper” throughout, ensuring that discussion is pertinent and the meeting ends on time.
5. Conducting the meeting efficiently and effectively, allowing each discipline the opportunity for input and questions and ensuring that the input is reflected with the MDS / CARE PLAN
6. Eliciting necessary and appropriate information from staff members to promote the development of problems / need statements, goal and approaches.
7. Responsibility for notifying family to invite them for care plan meeting.
8. Making sure that departments are aware of changes in resident’s conference.
9. Relating information to or from the conference in the occasional absence of staff members.
10. Ensure that all changes to MDS and CARE PLAN are done in a timely manner.
11. Transmitted MDS Assessment within guideline outlined by Medicare and Medicaid.
12. Making the Administrator aware of absence of staff members from conference.
13. Submit copies of each transmittal report to the Administrator / DON.
14. Discuss changes to MDS with each discipline as it relates to their area prior to changing codes.
15. Perform other duties as assigned by the Administrator or DON.
MDS Coordinator
Will be involved with residents, personnel, visitors, government agencies and personnel, etc. Be subject to hostile and emotionally upset residents, family members, etc. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
