Overview
Provide comprehensive medication management services including an evaluation of prescribed medications, over the counter drugs, herbal or nutritional products and immunizations for patients to ensure safe, effective utilization that is tailored to the individualized needs of each patient. Work directly with providers, nurses and other health care professionals to avoid problems associated with medication therapy. Identify and resolve medication therapy problems and accept accountability to achieve established goals of therapy. Documentation within local electronic medical record (EMR) or other MTM software as indicated.
This position will be split between on-site at M Health Fairview Fridley Clinic (0.6FTE) and remote supporting Medicare Part D population (0.4FTE).
Responsibilities/Job Description
- Provide Medication Therapy Management to patients as outlined in the MTM practice model.
- Ensure all drugs are appropriately indicated, effective, safe, and convenient for the patient.
- Obtain histories and apply these along with other patient data in the clinical evaluation.
- Identify, resolve, and prevent medication therapy problems.
- Evaluate the signs and symptoms relative to disease processes and make recommendations and/or modifications of medication therapy utilizing collaborative practice agreements developed and approved by the Fairview MTM program and M Health Fairview system.
- Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
- Ensure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goals.
- Evaluate and document patient outcomes at each encounter, as required by the MTM practice model.
- Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature and attending in-services, seminars, and conferences.
- Actively participate in department planning, goal setting, and implementation of those goals/plans. Actively participate in MTM and clinic meetings.
- Provide an excellent experiential site for pharmacy students and residents by training, mentoring and motivating students and residents as assigned.
- Maintain active preceptor status by MN Board of Pharmacy as required.

Qualifications
Education:
Minimum Education
- B.S. Degree in Pharmacy or PharmD from an accredited school of pharmacy.
Minimum License/Certification/Registration
- Current Minnesota pharmacy license.
- Certification in Pharmaceutical Care as approved by Fairview Pharmacy Services Management.
- Basic Life Support (BLS) Certification within 90 days of hire.
Preferred License/Certification/Registration
- Ambulatory Pharmacy Board Certification (BCACP), or other accredited board certification
Experience:
- What is the minimum level of experience needed to perform this job, if any?
- PGY1 residency or 3 years of experience as a practicing pharmacist in a clinical setting.
Preferred Experience
- If working in a specialty clinic, preferred experience with specialty population.
- Residency with an ambulatory focus.
