What are the responsibilities and job description for the Rehab Liaison/Intake Coordinator position at Fairview Health Services?
We are seeking a Rehab Liaision/Intake Coordinator to join M Health Fairview Rehabiliation Services! The coordinator will perform activities and duties related to recruitment, preadmission evaluation, education, and admission of patients to services along the rehabilitation continuum of care. For Acute Rehab Center, Transitional Care Unit, and Long Term Acute Care Hospital (LTACH) the liaision identifies potential patients, educates patients, family members, referral sources and payers, performs onsite or telephone assessments, coordinates smooth transition from referring site, recommend appropriate options for patients denied admissions.
- FTE 0.7, authorized for 56 hours per pay period.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Responsibilities Job Description
Identifies and assesses potential patients for admission to Acute Rehab, Transitional Care, and Long-Term Acute Care Hospital
Performs on-site and phone preadmission screening and clinical assessment
Provides recommendation as to appropriate level of post-acute care
Educates patient, family, referral sources, hospital staff, external payers as to rehabilitation program benefits and services
Maintains a thorough knowledge of facilities, services available
Serves as liaison case manager with internal and external case management teams
Assists with precertification processes by providing information/plan of care
Facilitates completion of preadmission screening in compliance with IRF regulation and preadmission clinical assessment in compliance with LTACH regulation
Contributes to completion of IRF-PAI
Facilitates seamless transition process for patient transfers
Serves as a consultant at off-site locations
Attends designated unit rounds to identify potential patient referral to Acute Rehab, Transitional Care, and Long-Term Acute Care Hospital
Reviews medical records to verify appropriateness of patients for admission
Offers options when acute rehab, transitional care, or LTACH is not indicated
Communicates effectively with referral sources including, but not limited to, the results of assessments, financial information etc.
Provides education to staff including, but not limited to, physicians, care coordinators, nursing, social services, therapists, direct care, and service providers, regarding acute rehab, transitional care, and LTACH
Continues to educate care providers on trends in health care, such as length of stay, alternatives to acute care, resource use, critical importance of discharge planning, etc.
Demonstrates ability to problem solve with staff on both sites, on issues delaying the transfer of patients and patient information
Continues to identify new referral sources
Promotes change and increases referrals to our program by educating medical staff and other health care professionals regarding effective hospital resources use and other options available
Collaborates with physicians, social workers, nurses, therapists, intake workers and other health care professionals to identify current utilization problems, educate regarding appropriate hospital resource use and facilitate case management
Assists the physician in preventing and/or appealing denials by educating regarding criteria for acute inpatient rehabilitation, transitional care, long term acute care hospital, alternative settings, adequate documentation, and the appeal process
Brings significant information to appropriate individuals or meetings in such a manner as to facilitate problem resolution and educate regarding appropriate hospital resource use
Collaborates with Facility Admission Coordinator to identify problems and facilitate reimbursement
Communicates appropriate information with others according to established procedures so that day-to-day operations are effectively conducted
Communicates, as needed, both written and verbal information in a clear, concise, accurate and considerate manner as observed or documented by peers or supervisor
Is accountable on an ongoing basis for information communicated at staff meetings discussing all unclear issues with supervisors
Interacts as needed, with co-workers in a respectful and cooperative way by communicating directly and by following proper channels of communication as observed by co-workers or supervisor
Qualifications
Required
3 years clinical experience, strong assessment, and evaluation skills
Licensed as a Registered Nurse or Rehabilitation Professional in State of Minnesota
Preferred
Bachelor’s Degree in a healthcare related field
4 years clinical experience, strong assessment, and evaluation skills
Excellent communication and interpersonal skills
Broad-based clinical knowledge
Demonstrates strong critical thinking
Strong patient assessment skills
Additional Requirements (must be obtained or completed within a period of time):
Certification in rehabilitation nursing or advanced training/certification in rehab related field completed within 18 months
3 or more years with discharge planning experience
Knowledge of regulations and requirements in post-acute care setting, including acute rehab, trans care, long-term care and long-term acute care hospital
