What are the responsibilities and job description for the Intensive Care Navigator position at Fairview Recovery Services, Inc.?
We are seeking a dedicated and compassionate individual to join our team as a Intensive Care Navigator. The role involves providing direct client services, crisis intervention, and supporting clients through recovery and other related services. The ideal candidate will have a strong passion for making a positive impact on individuals in need.
Key Responsibilities:
- Collaborate with an interdisciplinary team to plan, evaluate, and implement client direct services.
- Provide immediate supportive reinforcement to clients in crisis intervention situations.
- Offer outreach services to clients as needed.
- Liaise with external organizations, including probation, social services, courts, medical personnel, and other treatment agencies to support clients.
- Assist clients in accessing essential services such as temporary housing, childcare, and financial assistance.
- Help plan and implement educational, vocational, and training programs, while referring clients to appropriate schools, social services, and health organizations.
- Offer counseling on basic addiction, recovery, life skills, and available educational and vocational opportunities.
- Maintain accurate case records, progress reports, and other necessary documentation.
Minimum Qualifications:
One of the following:
- A bachelor’s degree from a regionally accredited or New York State registered college; or
- An associate’s degree in a human services field, plus at least one year of direct human services experience (preferably working with individuals with substance use backgrounds); or
- A high school diploma (or equivalent) with 6000 hours (about 3 years) of full-time, supervised experience in an OASAS-licensed, approved work setting.
- Completion of a supervised internship in a chemical dependency setting will count as one year of employment experience.
Special Requirements:
- Valid, clean driver’s license with fewer than three points (driving clearance is a mandatory requirement for this position).
