Faith Hospice is seeking a compassionate Registered Nurse (RN) or Social Worker (MSW) to lead our Hospice Community Team, focusing on delivering exceptional patient care and support.
Your Key Responsibilities
- Clinical Supervision: Supervise and guide an interdisciplinary team of professionals to ensure high-quality patient care.
- Care Planning: Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
- Quality Assurance: Monitor and maintain high standards of patient care, addressing any concerns promptly.
- Ongoing Support: Participate in the manager-on-call rotation to provide consistent support and guidance to your team.
Meet Our Requirements
- RN or MSW licensure in the State of Michigan.
- At least 1-2 years of management experience in skilled nursing or med/surg settings.
- Prior hospice nursing or social work experience is highly valued.
- Strong computer skills for EMR documentation.
