At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling.
What Will You Do?
In this community-based program, our current RN Case Managers :
Work a Monday – Friday daytime schedule
Participate in a back-up on call rotation with the other RNs on the team; this equates to the equivalent of one calendar week every 4 months (5pm – 10pm on weeknights and 9am – 9pm on Saturday and Sunday)
Manage a case load of hospice patients within an assigned geographic territory to minimize drive time and spend more time with your patients (we cover a 35-mile radius around Grand Rapids)

Education and Experience
RN licensure in Michigan
At least 2 years of med / surg or acute care nursing experience
Hospice and / or hospital experience preferred
Passion for hospice nursing
