Case Manager- DDP

Minimum Qualifications:

• Must be 21 years of age or older
• Bachelor’s Degree in a related field, as defined by DBHDD standards
• Minimum 1 year experience working with individuals with intellectual disabilities
• Must be able to read and write English proficiently.
• Bilingual in Spanish preferred.
• Excellent interpersonal and communication skills.
• Clean 7-year MVR and criminal record

Key Responsibilities:

• Provide training to staff on how to best meet the needs of residents, ensuring high-quality care.
• Conduct ongoing staff training and development on best practices.
• Guide and advise staff on serving individuals with disabilities.
• Supervise "at-risk" individuals, ensuring their safety and well-being.
• Oversee and monitor services and supports provided to individuals in our care, ensuring the delivery of services aligns with their Individual Service Plan (ISP).
• Participate in the development and review of ISPs, making necessary adjustments to meet the evolving needs of the participants.
• Supervise the formulation and delivery of all waiver services to the participant, ensuring that services align with their goals.
• Assist with the development and implementation of Crisis Plans, ensuring participant safety and health.
• Ensure compliance with health and safety strategies in collaboration with the appropriate team members.
• Complete all required training, including SIS, HRST, and ISP training through DBHDD.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
