The perfect candidate will identify and provide for each resident's social, emotional, and psychological needs, and the continuing development of their full potential. You must also be proficient in PCC and discharge planning with at least two years of experience in long term care. Pay starting at $60,000 depending on SNF experience with a $5,000 sign on bonus!
