Our Home Care and Foster Care programs provide assistance to residents throughout Fairfield County, and the Adoption program is both nationwide and international.

Position Summary :

The Adoption Assistance Program (AAP) is a confidential, home-based service offering assessments, education, brief counseling, and referrals for adoptive and guardianship families. We also provide support for professionals seeking assistance for their client families. This program uses an EAP (Employee Assistance Program) model, providing short-term services to address concerns impacting family stability. The role will serve families in the Greater Bridgeport, Greater Waterbury, and Greater New Haven areas and provide direct services to birth parents, adoptive parents, prospective adoptive parents, foster parents, and children as needed. Services include birth parent counseling, home studies, post-placement visits, and Permanency Placement Services (PPSP).

Key Responsibilities :

  • Conduct assessments and connect families with community resources
  • Lead parent support groups and advocate for children’s educational and developmental needs
  • Maintain detailed case notes and community resource lists
  • Provide birth parent counseling, home studies, post-placement visits, and adoption finalization support
  • Participate in statewide meetings and case planning

Qualifications :

  • Education : MSW or MS in a related field with two years of child welfare / adoption experience
  • LCSW preferred

  • Awareness of the cultural and socioeconomic characteristics of clients served.
  • Knowledge of the life cycle of adoption for individuals and families.
  • Experience conducting psycho-social assessments.
  • Excellent verbal and written communication.
  • Networking and community relations.
  • Organizing and maintaining client records.
  • Ability to work independently with children and families
  • Ability to work well within a team.
  • Experience : Knowledge of adoption life cycle and experience with psycho-social assessments
  • Experience in school advocacy and leading support groups
  • Excellent communication skills and ability to work independently and as part of a team
  • Additional Requirements :

  • Valid driver's license and access to a personal vehicle for regional and statewide travel.
