The Opportunity
The Families Count Program is seeking a Part-Time Case Manger to provide case management, family mediation and deliver the Active Parenting curriculum to families. The Case Manager will provide community networking to promote referrals and facilitate successful outcomes of the OCFS program.
Responsibilities
- Work collaboratively with community-based organizations, county, probation and faith-based organizations in addition to FCA programs, development, and social media.
- Ensure program operates in compliance with all contractual obligations and that all required paperwork, reports, and statistics are conducted, maintained, and submitted on time.
- Ensure that all services are strength-based and culturally competent.
- Maintain current knowledge and understanding of all Agency policies and procedures, regulatory requirements.
- Meet regularly with the program coordinator to communicate, on a timely basis, any issues and/or concerns affecting the program and its ability to comply with or attain the operational mission.
- Responsible for conducting monthly after-care contacts with former clients of the programs and wellness check on clients who have been non-compliant.
- Provide outreach to referrals and conduct intakes for new clients entering into the program.
- Maintain files, schedule appointments, and enter clients information into the OCFS database and 10e11.
- Assist Program Coordinator with group preparation and group facilitation.
- Additional responsibilities as assigned to ensure effective operations of the program.
Qualifications
- HS Diploma required, BA preferred
Minimum of 1 year of experience working with at-risk families and youth.
Excellent verbal and written communication skills - Proficient computer skills with Microsoft Office
- Bilingual in Spanish is preferred.
- Valid and clean License

Additional Information
- Salary: $20/Hour
- Schedule: (M-F), evening and weekend hours, as needed
- This role is part-time, 20 hours per week Mon-Fri, evenings and weekends as needed.
