What are the responsibilities and job description for the Per Diem Family Peer Advocate position at Family and Children's Association?
Position Summary:
Family Peer Advocates will work under the Nassau Family Center Program to provide client centered services and assist families living in Nassau County with children ages 5-18 that have been diagnosed with or at risk of being diagnosed with a Serious Emotional Disability. Family Peer Advocates will create a partnership with the families that is individualized to their needs and will build on family strengths to enhance their capacity to care for their child.
Essential Duties and Responsibilities:
- Conduct visits in the home and community setting to educate clients on parenting techniques and develop advocacy skills.
- Assist families in securing community resources to strengthen their support system.
- Facilitate groups, family engagement activities or other networking opportunities for clients.
- Attend all required program, agency and regulatory training and meetings.
- Complete all required paperwork in accordance with funding and agency requirements and submit by required deadline.
- Complete development and safety plans, session notes, discharge plans and all other pertinent documentation within required timeframes.
- Collaborate with Case Manager on shared client cases to ensure quality delivery of services.
- Additional responsibilities as assigned to ensure the effective operation of the program.
Qualifications
- High school diploma or equivalent required, college degree preferred.
- Must obtain provisional certification within the first 6 months of employment and the professional credential within 18 months or apply for an extension if more time is needed.
- ‘Lived-experience’ as the parent or primary caregiver of a child with a social, emotional, behavioral, mental health, or developmental disability required.
- Intermediate Knowledge of Microsoft Office applications.
- Valid and clean driver's license required.
Additional Information
Schedule
- Hourly - hours to be determined but will not exceed 19 hours per week.
- Flexible schedule based on client's needs and availability, which will include evening hours.
- Salary: $22/hour
- Location: Garden City
