Family Centered Services of Alaska (FCSA) is a nonprofit 501 (c) (3) corporation founded in 1989 by Fairbanks, Alaska community members committed to helping children with a wide spectrum of behavioral and mental health needs. FCSA provides behavioral and mental health services to youth aged 5-19. Services include outpatient therapy, education, foster care, and short or long-term residential treatment. FCSA is nationally accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and was one of the first organizations to be licensed by the state to establish, recruit, and monitor foster homes.
Smart Start Learning Center is a subsidiary of FCSA and provides daytime care to families in our community. Employees of FCSA are eligible to enroll their children in childcare services and enjoy a discount on their monthly tuition (contingent on enrollment availability).
About the Job
Family Centered Services of Alaska is seeking a Case Manager to oversee delivery of services and facilitate treatment to clients in our Foster Care Program. The Case Manager is responsible for maintaining the philosophy and effectiveness of the Therapeutic Family Group Homes program in Fairbanks, approval of individualized service plans, and facilitation of treatment team meetings. This position requires the use of your personal vehicle and vehicle insurance. Also required is the proficient use of the company’s Electronic Medical Records System. Newly hired employees will be subject to a review of their usage of this system after 60 days of employment.
Roles & Responsibilities
- Facilitate family and inter-agency collaboration.
- Advocate for the rights and needs of the youth and act as a community liaison who will represent FCSA to the professional and social community.
- Prepare client treatment plan.
- Monitor preparedness for client discharge.
- Monitor behavior and progress in relation to the treatment plan for each FCSA client.
- Identify and assist in remediation of “gaps” in the plan of care for each youth, create appropriate services when critical need cannot be met more traditionally, and coordinate any alternate treatment placement arrangements for the client, as needed.
- Review and process all Treatment Related Occurrence Forms (TROFs).
- Contact parents/guardians regularly to discuss major issues/progress.
- Coordinate travel arrangements for in and outgoing clients/families.
- Coordinate on/and off site visits, supervised phone calls.
- Schedule treatment team meetings in accordance with Quality Assurance timelines and standards.
- Create and update emergency binders at minimum weekly.
- Maintain all FCSA paperwork in accordance with FCSA Quality Assurance standards and timelines.
- Ensure all client documentation is entered into the EMR in accordance with FCSA Quality Assurance standards and timelines.
- On-Call –Rotation as scheduled.
- Other duties as assigned.
Education & Qualifications
- Bachelor's degree or equivalent work experience in psychology, social work, justice, guidance and counseling, sociology, education, nursing, or other human service field preferred.
- High school diploma or GED, and two years working with adolescents in a mental health setting required.
- Ability to work independently and make decisions with minimal supervision.
- Ability to communicate in a professional manner with appropriate stakeholders (Treatment Team Members, outside agencies, program director).
- Effective office skills including; time management, computer proficiency, strong verbal and written skills, and ability to complete required documentation in a timely/accurate manner.
- Physical ability to employ verbal and physical aggression control techniques after being certified.
- Knowledge of State of Alaska childcare regulations and juvenile justice laws.
- Freedom from previous criminal convictions that may adversely affect the employee's potential performance contingent on a fingerprint and/or records check.
- Current Alaska driver's license, driving record and vehicle insurance.
- Current first aid and CPR certificate.
This position is a driving position. Applicants will be required to submit a driving record if selected for the position.
All applicants must be at least 21 years of age due to State of AK regulations and must pass a State of AK Background Check.
