What are the responsibilities and job description for the Patient Coordinator position at Family Health Associates?
1. Greet patients and their representatives or guests professionally, both in person and on the phone
2. Guide patients through check in and check out processes, including arranging for follow up and other future appointments when appropriate
3. Communicate directly with patients and their representatives in person, by telephone, via the patient portal and through other means
4. Coordinate communication between patients and FHA staff
5. Quickly and efficiently answer or properly refer patient questions or issues to appropriate staff and follow up to make sure needs were appropriately addressed
6. Promptly notify clinical or other staff of patient arrivals
7. Comfort patients by anticipating anxieties and effectively answering questions
8. Help ensure appropriate care for patients by maintaining the accuracy of patient records
9. Ensure patient confidentiality, and provide patients with needed information
10. Monitor waiting room to make sure that everyone’s needs are being addressed
11. Work cooperatively and supportively with all other clinical staff
12. Manage provider schedules, make appointments and maintain smooth patient flow throughout clinic
13. Collect and maintain patient registration and utilization data as needed (i.e. insurance information, ID, patient surveys)
14. Perform other duties as assigned by designated FHA administration
Job Type: Full-time
Pay: $16.00 - $21.00 per hour
