Requirements
To be successful in this role, you must have:
- Baccalaureate or master's program in physical therapy approved by an accredited organization
- Current Illinois license as a Physical Therapist
- Current CPR card
- Valid IL driver's license and auto insurance policy
- Home Care Home Base experience preferred
