Family Home Health Network , an Affiliate of the Alden Network , is a provider of intermittent Home Health care, palliative care, hospice care, and skilled / custodial private care services throughout the Chicagoland area.
We are seeking a dedicated Hospice nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific hospice care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.
- This position is every weekend Saturday and Sunday 7am to 7pm. Will consider every other weekend.
