Patient Care Associate - Home Health

We are looking for Certified Nursing Assistants and Home Health Aides to join our home health team. The successful candidate will be responsible for providing quality care to patients in their homes, including monitoring vital signs and assisting with activities of daily living. The ideal candidate will have excellent communication and interpersonal skills, as well as a strong understanding of home health care protocols.

Responsibilities:

• Provide patient care services in a home health setting.
• Monitor vital signs and report changes to the nurse.
• Assist with activities of daily living, such as bathing, dressing, and grooming.
• Assist with ambulation and transfer of patients.
• Provide emotional support to patients and their families.
• Maintain a clean and safe environment for patients.
• Document patient care services in accordance with agency policy.

Job Types: Full-time, Part-time, Contract

Schedule:

• 12 hour shift
• 8 hour shift
• Day shift
