The Care Coordinator is responsible for helping LTSS - Long Term Support Services, including Visiting Volunteers, care recipients and their families to navigate and access community services and other resources. Services provided include but are not limited to connections to resources such as food, stable and safe housing, transportation, health education and social connection. Works collaboratively within a team structure.
QUALIFICATIONS
· High school diploma or equivalent (required).
· Certified Community Health Worker, VA Department of Health (required unless you have a BSW or MSW).
· Experience as a Community Health Worker or Care Coordinator (preferred).
· Excellent interpersonal and public relations skills.
· Excellent organizational and communication skills.
· Accurate documentation skills.
· Knowledge of Microsoft Office, with the ability to learn common Family Lifeline software applications, including web-based case management system.
· Able to maintain a flexible work schedule.
· Regular, predictable, and reliable onsite attendance required.
· Possess a valid Virginia driver’s license, automobile insurance, and reliable transportation.
· Able to pass a required background and drug screen check.
· Ability to speak and write in English.
ESSENTIAL RESPONSIBILITIES
· Establish a trusting relationship with care recipients and their families to assess their needs and make connections to appropriate referral sources.
· Conduct intake and service delivery according to Family Lifeline’s protocols.
· Knowledgeable about community resources appropriate to the needs of care recipients/families.
· Make home visits to provide assessment, health education, and social support.
· Provide culturally appropriate health education and information (e.g. conduct health promotion and disease prevention education in a manner that matches linguistic and cultural needs of care recipients/families).
· Provide care coordination, case management, and system navigation (e.g. makes referrals and provide follow-up, facilitate transportation services and helps to address other barriers to services).
· Coach and educate care recipients regarding health coverage and benefits.
· Act as an advocate and liaison between the care recipient/family and community service agencies.
· Document case notes and service plans in an accurate and timely manner.
· Conduct outreach activities to increase awareness of services and make presentations at local agencies and community events.
· Advocate for the needs of care recipients/families.
· Report all suspected incidents of abuse and neglect according to the laws of the Commonwealth of Virginia.
· Participate in organization-wide meetings, assigned meetings and activities.
· Assist with LTSS administrative duties as needed.
· Perform other duties as assigned.
