Family and Medical Counseling Service is seeking a QI Data-HIM Specialist to join our amazing team!

INTRODUCTION

Under the supervision of the QI-HIM Coordinator, the HIM Specialist will works collaboratively with the Quality Improvement (QI) Manager / Coordinator and Team to develop and implement CQI Plan and Health Information Management workflows and systems.

MAJOR DUTIES / ESSENTIAL FUNCTIONS

Essential and other important responsibilities and duties may include, but are not limited to the following :

  • Create, analyze, and use data to collaborate, and identify best practices and areas for improvement
  • Lead QI-Subcommittee and support staff with improving documentation to support data reporting
  • Conduct data analysis and research data sources to account for and make any corrections to existing data

as needed to ensure accuracy of the information being recorded

  • Extract and report data to meet local and federal program requirements, support agency objectives and
  • internal management decision making processes

  • Monitor and measure indicator and outcome measures to evaluate effectiveness of applied standards
  • Process Primary Medical, Pediatrics, and Behavioral Health incoming and outgoing medical record
  • request (i.e. Subpoenas, Court Orders, investigative reports, insurance company request, Medical

    Examiner reports, patient request, etc.) ensuring HIPPA compliance when processing request

  • Assist with managing agency and EMR fax inboxes processing documents and appropriately assigning to
  • staff and attaching to patient charts as needed

  • Follow-up with records for medical provider referrals and orders, and appropriately assign to Verisma to
  • attach to charts in the EMR and receive and scan documents from medical into EMR

  • Receive and distribute incoming USPS mail daily
  • Liaison with government agencies, hospitals, and other external parties to ensure correct and appropriate
  • sharing of confidential health information

  • Support and maintain the accuracy of patient demographic data in electronic health record
  • Handle phone and walk in inquiries from patients
  • Adhere to agency policy and procedures and maintain data integrity and abide by Patient Rights, and
  • Health Insurance Portability and Accountability Act (HIPPA) Privacy Standards

  • Attend relevant meetings to support overall program implementation
  • Attend trainings and learning opportunities that expand content knowledge and best practices
  • Other duties as assigned
  • QUALIFICATIONS, KNOWLEDGE & SKILL REQUIRED :

  • Bachelors degree in public health, health information or another applicable field
  • HIPAA Privacy Expert (CHPE) Certification
  • Electronic Health Record (eClinicalWorks) and reporting tool (ELOOM) experience
  • SQL Coding and Data Development and Reporting Experience
  • High degree of independence, flexibility, initiative and commitment
  • Strong analytic and problem solving and verbal and written communication skills
  • Detail-oriented and can consistently meet deadlines and work well within a team environment
  • Strong planning, organizational and critical thinking skills
  • Ability to prioritize work and multi-task
  • Ability to work collaboratively with physicians, staff and external organizations
  • 1-2 years experience with quality improvement and health care clinic operations
  • HOURS OF WORK :

    Regular hours between 8 : 30 AM 5 : 30 PM, Monday Friday, with option of Telework (up to 50%)

    Salary : $50 - $55
