**Job Summary:**
The Client Support Coordinator will provide habilitative assistance and care to clients in various Agency programs. This role involves direct daily care, teaching, and supervising of clients, along with record-keeping responsibilities. Passionate and caring is a must.
**Responsibilities:**
- Directly supervise clients, monitor their status and location at all times.
- Assist in preparing and implementing individual client plans.
- Teach, supervise, and assist clients in adult daily living activities.
- Monitor client progress on goals and protect client rights.
- Complete incident reports and medication error reports.
- Implement behavioral plans and therapist recommendations.
- Develop and implement recreation and community participation activities.
- Collaborate with Clinical Director, Consultants, families, and other agencies.
- Attend meetings, in-services, and required trainings.
- Administer medications, provide routine health care, and first aid.
- Maintain cleanliness and organization of program sites and equipment.
- Transport clients to designated activities and programs.
- Participate in emergency drills and maintain medication certification.
- Keep accurate records of services provided to clients.
**Qualifications:**
- Successful completion of Medication Certification Program.
- Successful completion of PMT, CPR, and First Aid training programs.
- Valid CT driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals with developmental and/or behavioral challenges preferred, or equivalent combination of education and experience.
**Physical Demands:**
- Regularly talk, hear, stand, walk, sit, use hands, and reach.
- Lift and move up to 75 pounds and exert over 100 lbs. of force if needed for physical restraint procedures.
- Specific vision abilities required, including close vision and ability to adjust focus.
**Language Skills:**
- Ability to read and interpret English documents, write routine reports, and correspondence.
**Mathematical Skills:**
- Ability to add, subtract, multiply, and divide using whole numbers, fractions, and decimals.
**Reasoning Ability:**
- Apply common sense to carry out detailed instructions and deal with problems involving concrete variables.
