Job Summary:
The Client Support Coordinator will provide habilitative assistance and care to clients living in various Agency programs, including community and supported living arrangements, family support services program, and day/vocational program. The Coordinator will be responsible for the direct daily care, teaching, and supervising of clients, as well as record keeping. Showing patience and caring for our residents. To perform any other related duties as requested by the Program Director or the Program Manager.
Responsibilities:
- Supervise clients, ensuring knowledge of their status and location at all times.
- Assist in the preparation and implementation of each individual client's Individualized Plan of Service or Individual Educational Plan.
- Teach, supervise, and assist clients in performing adult daily living activities such as hygiene, personal care, eating, banking, shopping, meal-planning, clothing planning and care, community participation, and self-control.
- Monitor client progress on goals and report incidents of abuse, neglect, and mistreatment.
- Complete incident reports, medication error reports, and implement behavioral plans as written.
- Implement recommendations and programs from occupational, physical, and speech therapists.
- Develop and implement recreation and community participation activities for clients.
- Collaborate with Clinical Director, Consultants, families, schools, and other agencies to provide and implement goals.
- Maintain accurate and timely record keeping of services provided to clients.
- Work cooperatively with supervisory and other staff within the Agency and other agencies.
- Attend required staff meetings, in-services, and trainings.
- Administer medications, provide routine health care and first aid to clients.
- Ensure cleanliness and organization of program sites and transportation vehicles.
- Transport clients to and from designated activities and programs.
- Conduct and participate in emergency drills as required.
- Complete and maintain medication certification.
- Ensure accurate recording keeping of clients' financial accounts.
- Perform any other related duties as requested by the Program Director or Assistant.
Qualifications:
- Must be able to complete the Medication Certification Program.
- Must be able to complete PMT, CPR, and First Aid training program.
- Valid CT driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Minimum of one year of experience working with individuals with developmental and/or behavioral challenges preferred, or equivalent combination of education and experience. Will train right applicant.
Physical Demands:
- Ability to lift and/or move up to 75 pounds.
- Ability to exert force in excess of 100 lbs. when initiating physical restraint procedures.
- Ability to stand, walk, sit, use hands and arms, talk, and hear.
- Specific vision abilities required, including close vision and the ability to adjust focus.
Language Skills:
- Ability to read, interpret, and write routine reports and correspondence in English.
Mathematical Skills:
- Ability to perform basic mathematical calculations.
Reasoning Ability:
- Ability to apply common sense understanding and problem-solving skills.
Work Environment:
The work environment is usually moderate in noise levels. The employee may be required to travel by automobile and be exposed to changing weather conditions.
