Job Summary:
The Client Support Coordinator (CSC) is responsible for providing habilitative assistance and care to clients in various Agency programs. This includes community and supported living arrangements, family support services program, and day/vocational program. The CSC will be responsible for the direct daily care, teaching, and supervising of clients, as well as maintaining accurate record keeping.
Responsibilities:
- Supervise clients, ensuring their safety and knowing their status and location at all times.
- Assist in the preparation and implementation of each individual client's Individualized Plan of Service or Individual Educational Plan.
- Teach, supervise, and assist clients in performing adult daily living activities, such as personal care, eating, banking, shopping, and community participation.
- Monitor client progress on goals and ensure client rights are protected.
- Complete incident reports and medication error reports when necessary.
- Implement behavioral plans and approved or emergency restraints and procedures as needed.
- Implement recommendations and programs from occupational, physical, and speech therapists.
- Develop and implement recreation and community participation activities for clients.
- Collaborate with Clinical Director, Consultants, families, schools, and other agencies to provide and implement goals.
- Maintain accurate and timely record keeping of services provided and specific goals identified for each client.
- Work cooperatively with supervisory and other staff within the Agency and from other agencies.
- Attend required staff meetings, in-services, and trainings.
- Administer medications, provide routine health care, and first aid to clients.
- Keep program sites and equipment clean and organized.
- Transport clients to and from designated activities and programs.
- Conduct and participate in emergency drills.
- Maintain medication certification and accurate recording keeping of cash accounts.
- Perform other related duties as requested by the Program Director or Assistant.
Qualifications:
- Successful completion of Medication Certification Program.
- Successful completion of PMT, CPR, and First Aid training.
- Valid CT driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals with developmental and/or behavioral challenges preferred, or an equivalent combination of education and experience.
Physical Demands:
- Regular standing, walking, sitting, and reaching with hands and arms.
- Ability to lift and/or move up to 75 pounds, and exert up to 100 lbs. of force when initiating physical restraint procedures.
- Close vision and ability to adjust focus required.
Language Skills:
- Ability to read, interpret, and write routine reports and correspondence in English.
Mathematical Skills:
- Ability to perform basic mathematical calculations.
Reasoning Ability:
- Ability to apply common sense and problem-solving skills to carry out instructions and handle various situations.
Work Environment:
The work environment is usually moderate in terms of noise level. The CSC may travel by automobile and be exposed to changing weather conditions.
