Reference # : 2828979 Description Job title Life Skills Case Manager Reports to Program Director Program overview The Life Skills Case Manager for the mainstream Program provides supportive service to formerly homeless mental health consumers who may be in recovery from substances who are living in housing owned by Columbus Property Management. Mainstream is housed in with the Fasst / Connections program. Position Overview includes the following, but not limited to

• Screen new applicants, complete initial assessments, and other intake forms.
• Provide at least bi-weekly face to face contact with residents, and weekly phone calls.
• Write progress notes and fully maintain client's charts.
• Complete monthly service plans and update release forms as needed.
• Assist residents in the development and completion of strength-based goal plans.
• Develop and facilitate special focus workshops for residents and conduct monthly community meetings with the residents.
• Identify and participate in life skills outings for consumers to enhance the quality of life and independent living in the community.
• Monitor and inspect residences as needed and maintain a collaborative relationship with Columbus Property Management and other service providers. Benefits of working for RHD Mainstream

• Assess and evaluate the needs of participants and provide linkages to appropriate services.
• Monitor service delivery by maintaining regular contact with participants and service providers / programs involved with the participant.
• Collaborate with participants utilizing their own individualized strength-based personal goal plan reviewing and modifying as necessary.
• Regular home visits, as well as accompany participants to medical appointments and treatment facilities as appropriate.
• Educate, inform, and advocate for participants regarding benefits and entitlements, treatment, self-advocacy, and natural supports. Skills
• Understanding and respect for each individual's unique path to recovery. Able to provide trauma-informed, strength based support.
• Demonstrate professionalism and ethical service at all times.
• Able to work well independently and as part of a team.
• Excellent time management and flexibility.
• Must have good verbal and written communication skills.
• Computer literacy is essential. Must be able to effectively use electronic health record, video conferencing, email, and any other technology needed to meet job responsibilities
• Must have adequate knowledge to navigate the City of Philadelphia via car or public transit.Requirements Required Experience 0-1 year experience working in an environment with Mental Health illnesses is preferred. Required Education Bachelor's degree with major course work in one of the following areas : Sociology, social work, psychology, criminal justice or Counseling. Required Qualifications
• Knowledge of addictions, residential facilities, mental illnesses and their tre tment.
• Knowledge in crisis prevention and de-escalation.
• A valid drivers license is required RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date).
