We are an Affiliate of Family Promise, the leading organization dedicated to preventing and ending homelessness for families with children. We seek an organized, compassionate, and motivated teammate to help families quickly transition from homelessness to permanent housing, leveraging Family Promise’s trauma-informed, low-barrier, and housing-first approach. The Case Manager will also assist families in reaching goals they set for themselves, including increasing earning potential, youth-specific goals, and educational goals. You will be responsible for our Rapid Rehousing Program. However, you will be cross-trained on all programs and may assist with them as needed.

Location: Hybrid. Our office is located at 17527 Nassau Commons Blvd, Lewes DE, 19958. Must be located in the Southern Delaware region and be willing to travel during the day throughout Sussex County to meet clients as needed in the area.
Overall Job Objective:
Rapid Re-Housing (RRH) provides temporary rental assistance to support families in housing with the goal of self-sustainability. The RRH Case Manager will be responsible for promptly assisting families experiencing homelessness in finding housing. The person will work with program participants to eliminate barriers to housing stability, including employment, increasing income, addressing mental/physical health, transportation, childcare, and community engagement. The successful candidate will be able to manage up to 20 family cases at once, build on participants' strengths, develop person-centered service plans, and connect participants with needed resources and services. Creative problem-solving, careful record-keeping, and developing strong community/landlord linkages and relationships will be key.
Job Duties
This position entails the following responsibilities and may include other duties as assigned:
· Assess program participants at program entry to jointly develop a strengths-based, goal-driven action plan that promotes permanent housing and self-sufficiency in the future.
· Identify participant strengths and barriers to stability to help ensure proper housing options.
· Work with participants to locate and secure appropriate housing; assist with landlord negotiations, housing applications, and leases when needed; advocate with landlords for prospective tenants.
· Develop a referral network of services and supports that assist with housing retention, including linkages for warm handoffs to specialists in employment, transportation, childcare, social service benefits, substance use issues, mental health, etc.
· Provide comprehensive case management support to program participants and ensure services are individualized, culturally competent, family-focused, goal-driven, strengths-based, and appropriate.
· Match program participants to appropriate housing options, always considering their needs and ability to generate income.
· Build relationships with program participants to learn their needs, goals, strengths, and barriers to maintaining stable housing. Keep communication channels open and be proactive about problem-solving.
· Maintain records and notes for each participant, track and report participant outcomes in the Client Management Information System (CMIS) and organization database, and provide monthly progress reports to the supervisor.
· Build feasible, sustainable action plans with participants to help them locate and maintain housing and break the cycle of homelessness.
· Prepare program participants for eventual self-sufficiency:
o Build connections with potential employers and training programs.
o Prepare participants for job interviews; assist with building a resume.
o Teach participants budgeting and financial literacy skills.
o Ensure participants possess basic life and home care skills; provide training, if needed.
· Identify and provide outreach to landlords and community organizations to educate them about RRH and related services:
o Locate suitable RRH housing options.
o Build relationships and connections with landlords.
o Serve as a landlord liaison to help mitigate any issues that arise with tenants.
o Work with tenants and landlords to solve issues and prevent eviction.
· Conduct visits in participants’ homes once housed to ensure compliance with lease and program agreements.
· Attend team meetings and training.

Education Level
Bachelor’s degree required.
At least 2 years of case management experience preferred. Paid or volunteer experience working with people experiencing homelessness or in health and social service settings such as foster care, behavioral health, domestic violence, community health centers, etc. Lived experience of homelessness, trauma, or behavioral health challenges is welcome.
