Position Overview: A Stabilization Case Manager provides essential support to families who have moved from our shelter programs into permanent housing. You’ll work within a collaborative, team-oriented environment to guide families toward stability, equipping them with the tools and skills necessary to maintain independence and success in their new homes.
This position is grounded in Trauma-Informed Care and Housing-First practices, ensuring dignity and empowerment for every family we serve.
What You’ll Do
- Build genuine, trusting relationships with families, offering support as they navigate the challenges of maintaining stable housing.
- Implement case plans tailored to each family’s goals and needs, meeting with them regularly.
- Manage thorough case notes, ensuring accurate documentation and accountability.
- Assist families in learning critical household management skills and navigating resources to remain stably housed.
- Collaborate with landlords to secure housing solutions when needed.
- Maintain strict confidentiality, professional boundaries, and compliance with mandatory reporting laws.
- Partner with community organizations to develop new resources and strengthen existing networks.
- Participate in staff meetings, trainings, and community events to enhance professional growth and team cohesion.
What We’re Looking For
- Education:
- High School Diploma or GED (required).
- Bachelor’s degree in Social Services or a related field (preferred).
- Certifications in de-escalation or applicable areas are a plus.
- Experience:
- At least 2 years working with children and families, especially within diverse populations or those experiencing homelessness.
- Specialization in areas such as housing, employment, or child and family resources is valued.
- Skills and Attributes:
- Compassionate with a strong sense of social awareness.
- Adaptable in dynamic, sometimes chaotic environments.
- Self-motivated and capable of managing multiple priorities.
- Proficient communicator, both written and verbal.
- Professional, emotionally mature, and team-oriented.
Work Environment & Physical Requirements
- Based at our Mission Avenue location, with frequent visits to family homes.
- Active mobility is required for up to 2 hours at a time, including moving and lifting up to 25 pounds.
- Work in an environment that can be busy, loud, or unpredictable.
