PERMANENT SUPPORTIVE HOUSING CASE MANAGER
POSITION ANNOUNCEMENT
Family Promise of the Big Bend is seeking a Permanent Supportive Housing care manager. A successful candidate will not only fulfill qualifications and demonstrate required skills, but also bring a trauma-informed awareness to our strength-based organization. The ideal candidate will demonstrate an understanding of and commitment to Housing First principles. The ability to network with local service providers, use de-escalation and conflict resolution skills, and keep a client-centered focus, is required along with a value for diversity, a willingness to self-assess, and the capacity to adapt to change and context.
PRIMARY RESPONSIBILITIES
· Provide care management and crisis intervention for a caseload to support clients with housing stability
· Assess needs, coordinate with team members, and assist with housing move-ins
· Develop, implement, and monitor Individualized Housing Plan (IHP) that is client centered, in partnership with program participant
· Provide housing-related information, education, and referrals related to housing stability and basic needs
· Provide in-home visits for initial intake and ongoing follow-up visits
· Provide advocacy on behalf of clients
· Demonstrate knowledge of community partners and ability to maintain cooperative working relationships with these partners
· Coordinate local supportive services with program participant and community partners
QUALIFICATIONS & SKILLS REQUIRED
· Bachelor’s in Social Work or a related Human Services degree
· Excellent interpersonal skills and demonstrated ability to work with marginalized or underserved client populations, e.g., experiencing homelessness
· Ability to foster working relationships with colleagues, clients, and community agencies
· High level of professionalism and ability to work independently
· Strong organizational, time management and communication skills
· Computer proficient
· Crisis management experience
JOB TYPE: Full time (position will require local travel and occasional work hours outside of regular M-F work week)
PAY RANGE: $38,000-$40,000
