What are the responsibilities and job description for the Community Habilitation Specialist position at Family Residences and Essential Enterprises?
Overview
The primary role of the Community Habilitation Specialist is to work alongside the people we support in order to help them to achieve their Personal Outcomes. This includes working with individuals with special needs to develop daily living, professional and decision making skills while also supporting their health and safety.

Responsibilities
- Provides direct services that support the participant(s) in a least restrictive manner.
- Teach, assist, and motivate the participant(s) in identifying activities that reflect the participant(s) wishes based on the participants objectives and valued outcomes identified in the Individual Service Plan (ISP) and assist them with developing more independence in those areas.
- Responsible for ensuring the supervision of participant(s) based on individuals abilities including areas of self-care, independent living, community integration, etc.
- Completing documentation of services provided in accordance with regulations, policies, practices and procedures.
- Respond appropriately to crisis/emergency situations in order to ensure and secure the safety and well-being of participant(s).
Qualifications
- Minimum of 1 year working with individuals diagnosed with developmental and/or intellectual disabilities
- Valid NYS Driver’s License and meets the qualifications to be a driver for the agency
- High School Diploma/GED or equivalent
