Felton offers a dynamic, challenging, and supportive work environment where we encourage the use of professional development, that staff can utilize throughout their career.Program DescriptionYoung Adult Court (YAC) in San Francisco is part of the larger Collaborative Court system and serves eligible young adults ages 18-24 for an average of 18 months. Collaborative partner agencies include the Superior Court, Office of the District Attorney, Office of the Public Defender, the Department of Public Health, Adult Probation Department, Department of Children, Youth and their Families, and Felton Institute.The clinical team assesses the risks and strengths of participants and works collaboratively with clients and the court to develop individualized Wellness Care Plans. These plans address mental health, substance use, housing support, physical healthcare, benefits acquisition, employment services, and more. The court uses these care plans to track client progress and accountability throughout the program.Job DescriptionThe Clinical Case Manager (CCM) provides comprehensive support to justice-involved transition-age youth (TAY). The ideal candidate will offer clinical services to address a range of mental health, substance use, and criminogenic needs using a strengths-based, client-centered approach rooted in harm reduction. This position requires flexibility, cultural humility, and the ability to adapt to the evolving needs of clients in a fast-paced environment.In addition to providing integrated mental health and substance use support through individual and group interventions, including Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT) skills, Motivational Interviewing, and life skills training, the CCM fosters a welcoming, safe, supportive, and culturally affirming environment for participants.This role includes maintaining accurate documentation, managing data entry, and generating reports for collaborative partners, ensuring continuity of care. The position is ideal for a professional who can balance therapy, case management, client accountability and community collaboration to support justice-involved individuals effectively.Program Specific ResponsibilitiesConduct Assessments : Perform bio-psycho-social and criminogenic screening interviews, both in and out of custody, and complete regular diagnostic evaluations, written assessments, and reports.Provide Therapy and Treatment : Deliver individual therapy and treatment services to address substance use and mental health challenges, including depression, anxiety, complex trauma, and addiction. All services are provided within a harm reduction framework, emphasizing a strengths-based, client-centered approach.Flexible Case Management : Provide intensive case management for a caseload of approximately 15-17 participants, with varying needs and levels of intensity. Work collaboratively with community providers to promote client stability, recovery, and access to mental health and healthcare supports.Group Facilitation : Plan and facilitate weekly in-person skills groups, which may include curriculum development tailored to the needs of the client population.Evidence-Based Practice : Demonstrate proficiency (or willingness to receive training) in Motivational Interviewing, Cognitive Behavioral Therapy, and other evidence-based practices that support diverse TAY populations.Wellness Care Plans : Develop and implement individualized Wellness Care Plans in collaboration with participants, community partners, and the court.Culturally Responsive Support : Provide participants with access to culturally appropriate community-based resources. Emphasize cultural humility by recognizing and respecting the diverse backgrounds and experiences of clients in all interactions.Documentation and Compliance : Ensure all documentation, reports, and deadlines are met while maintaining compliance with program requirements as well as laws about mandated reporting and compliance. Monitor and track participants’ progress in achieving their goals and objectives.Technical Skills : Manage multiple databases efficiently and utilize the Office Suite and cloud-based file storage for documentation.Supervision and Self-Reflection : Participate in weekly clinical supervision, demonstrating self-awareness, introspection, and the ability to both give and receive constructive feedback, while managing transference and / or countertransference.Licensing Compliance : If registered with the BBS, manage all BBS paperwork and meet renewal requirements.Collaboration : Work collaboratively with all players involved in the client’s care, including community partners and members of the court, even when there may be disagreements regarding case management recommendations.Other Duties : Perform additional duties as required.Minimum QualificationsMaster’s degree in social work, Counseling Psychology, or related fields of clinical behavioral sciences.Licensed or license-eligible preferred.Minimum of 2 years experience providing mental health treatment and / or case management for young adults, including experience working with individuals with co-occurring disorders.Must be able to acquire jail clearance in the City and County of San Francisco and accessing a SF Superior Court database; requires passing the Sheriff’s Department and Superior Court’s LiveScan process.Comprehensive range of mental health and substance use counseling and crisis intervention skills.Demonstrates understanding of diagnosis referenced in most current Diagnostic and Statistical Manual and how diagnostic symptoms may impact a client’s ability to engage in YAC program requirements.Strong organizational, communication, interpersonal, and time-management skills.Ability to maintain accurate and timely documentation of clinical work.Exhibits understanding of developmental stages, with a focus on Transitional Age Youth.Experience working with TAY who are justice impacted and court diversion programs a plus.Ability to work independently and collaboratively within an interdisciplinary team.Knowledge of San Francisco / Bay Area TAY services and resources; familiarity with multi-cultural populations, especially African-American, Asian and Latino cultures.Bi-lingual (Spanish / English) strongly encouraged.Ability to manage transportation to and from county jails and other off-site locations.Additional Information : Reports to : Clinical Director of Innovation and DevelopmentJob Type : Full-TimeWork Schedule : OnsiteFLSA Status : ExemptSalary Range : $75,000 - $90,000 Annually (DOE)Benefits (subject to election and eligibility) : Fully-Paid Medical and Dental Plans; Health Reimbursement Card, Paid Time Off (personal, vacation, and sick time), 403B with company match.Applying Instruction : Candidates should apply for the position at our career page “careers.felton.org” OR directly email resumes to “jobs@felton.org”.IMP NOTE : Job responsibilities are subject to change according to program requirements.#J-18808-Ljbffr
