I. JOB SUMMARY

Family Service and Children’s Aid Society employs the position of Youth Connection Program Coordinator to provide match support to ensure child safety, positive impacts for youth, constructive and satisfying relationships between children and volunteers.

The Program Coordinator’s schedule varies and will provide service for the youth matched and on the waiting list for individuals living in Venango County. Program Coordinator will assist the Program Director with support for youth and mentors also located in Clarion, Forest and Eastern Crawford Counties. The hours are determined by consumer and program needs, including evening and weekend hours as required.

The position reports directly to the Youth Connection Program Director with formal individual supervision being held on a regular basis and informal supervision being available on an ongoing basis. This position also reports indirectly to the Chief Executive Officer of Family Service and Children’s Aid Society. Employees are expected to comply with all programs, Agency, county, and state requirements.

II. ACCOUNTABILITIES REGARDING WORK PERFORMED

1. Direct Service

A. Through scheduled in-person, telephone, and electronic contact, to assure that the elements of child safety, match relationship development, positive youth development, and volunteer satisfaction are fulfilled, and that potential problems and barriers are identified and addressed as early as possible. Promptly notify Youth Connection Program Director regarding concerns, which may negatively impact the match.

B. Document, monitor, and complete all elements of match support, supervision, and survey administration identified by the Agency, Mentor Core, or Program Director.

C. Ensure high-level proficiency in applying child safety and risk management knowledge, policies, and procedures throughout all aspects of job function.

D. Develop strategic interventions to identify and strengthen match relationships that require extra support to continue to grow.

E. Establish, monitor, and meet goals for match length and customer satisfaction. Utilizing Mentor Core survey’s, assess match impact on youth development.

F. Recruit potential mentors and mentees for Venango County. Assist the Program Director with recruitment in Clarion, Forest and Eastern Crawford Counties.

G. Complete Venango County enrollments, including individual and/or group orientation, volunteer training, child safety education, interviews and completion of any other enrollment processes.

H. Conduct exit interviews with all parties at match closure. Assesses reasons for match closure and re-match potential. When a match terminates prematurely or unexpectedly refer exit interview to the Youth Connection Program Director for completion.

I. Share with the Program Director potential partnership relationships as discovered through volunteers’ and parents’ employers and affiliations.

J. Identify and promote engagement of volunteers as Mentors, committee members, donors, and in other volunteer capacities.

K. Provide timely and comprehensive written summaries of all contacts utilizing the supervision and case notes sections in Mentor Core.

L. Participate in program sponsored activities within the community and assist the Program Director in providing activities for Matched Mentees and children on the wait list.

2. Record Keeping

A. Collect, review, and report all required statistical data to the Program Director related to the services being provided in order to give an up-to-date profile of service activities.

B. Compile statistical data to be used by the Program Director to comply with program standards and to acquire additional funding.

C. Monitor program consumer files in order to ensure complete record of service activities, to be accountable for services delivered, and to ensure that the required forms and documents have been secured.

D. Update the Match Participant Sheet as needed.

E. Review the Match Support Calendar for monthly contacts and update as contacts are completed.

F. Complete the Stat sheet at the end of each month ensuring contacts have been recorded in the county in which they occurred. Forward the completed Stat report to the Program Director.

3. Public Relations

A. Provide community presentations throughout Venango, Clarion, Forest and Eastern Crawford Counties regarding the scope of services provided to better inform the community of services relative to the programmatic system.

B. Attend the monthly Venango County Focus on our Future (FOOF) meeting to network with other local agencies and to promote the Youth Connection program.

C. Update and maintain the Youth Connection Facebook Page.

D. Distribute event flyers and Program brochures on an as needed basis.

4. Other

A. Attend training sessions, conferences, meetings and in-services in order to keep abreast of current information, regulations, and general knowledge to enhance professional growth.

B. Assist the Program Director and Youth Connection Advisory Committee with duties and activities related to fundraising. Assist with the design, completion and distribution of the Youth Connection annual Lottery Calendar.

C. Attend the monthly Youth Connection Advisory Committee meeting and if need be, fill in for the Program Director in the event of his/her absence.

D. Perform other duties deemed appropriate by the Program Director or Agency Executive staff to enhance the operation of the Youth Connection Program or of the agency as a whole.

III. HIPAA MINIMUM NECESSARY STANDARD

The minimum necessary standard, a key protection of the HIPAA Privacy Rule, is derived from confidentiality codes and practices in common use today. The minimum necessary standard requires the Agency to evaluate our practices and enhance safeguards as needed to limit unnecessary or inappropriate access to and disclosure of protected health information.

This position may only access the minimum information that is necessary to perform all job accountabilities included in this job description.

1. Permitted Accesses

A. Access to consumer files for the Youth Connection Program.

B. Knowledge of limited Protected Health Information of consumers in other Agency programs.

2. Justification/Purpose

C. To carry out the operations of the Youth Connection Program including, compiling consumer files to ensure a complete record of service activities, and maintaining accurate consumer files in order to comply with program standards.

IV. REQUIREMENTS

1. Act 33, 73, 34 clearances.

2. Valid PA driver’s license and reliable transportation to use on the job when necessary and valid proof of insurance if personal vehicle is used for business purposes.

3. Ability and commitment to uphold and maintain confidentiality.

4. Availability and willingness to work evenings and weekends as required by program needs.

5. A willingness to attend out-of-town conferences, training workshops, and meetings as necessary.

6. Ability to undergo and successfully pass a pre-employment drug screening.

V. QUALIFICATIONS

1. Ability to communicate in a clear, concise, and professional manner.

4. Ability and personality characteristics to work effectively with people from all social, economic, ethnic, educational backgrounds and age groups.

11. Ability to respect and practice confidentiality.

12. Knowledge of the interview process, human behavior, and social work ethics.

13. Ability to maintain the case manual.

14. Ability to provide outstanding customer service.

VI. MINIMUM EXPERIENCE AND TRAINING

· Preferred: Bachelor’s degree in social services, mental health or related human services field.

· Preferred: One year of experience working with both child and adult populations, along with familiarity with intake assessments and interview processes.

Job Type: Full-time

Pay: $16.50 per hour

Expected hours: No less than 37.50 per week
