Work Schedule : Monday thru Friday
Varied hours between 7 am to 7 pm; must be able and willing to adjust work schedules based on their clinical case load and the needs of individual clients.
Position Summary
Wraparound Facilitation (WAF) services are designed to provide medically necessary services to the serious emotional disturbance (SED) client (children / youth) to assist them in resolving or minimizing the effects of the mental and emotional impairment impacting their daily lives. Wraparound services are provided under a treatment plan approved by a psychiatrist, physician, or qualified mental health provider skilled in the treatment of mental disorders and are for qualifying SED children / youth that are also in the SED Waiver program. The position will work in the office and community. The goal is to enhance independent functioning through which the client is integrated into the community and is able to maintain independent functioning, so that institutionalization is not as likely or as frequent. The client and wraparound team work toward this goal by developing and following a plan of care which relates back to specific goals set forth in the consumers’ treatment plan.
ESSENTIAL FUNCTIONS
- Provide direct services to serious emotional disturbance (SED) children / youth and their families.
- Provide direct services based on the philosophies and principles of the strengths model.
- Complete required trainings for position.
- Forms the Wraparound team which consists of client’s family, extended family, and other community members involved in the client’s daily life. Will schedule and facilitate the Wraparound meeting and assist with the development of the person centered service plan by guiding the team through assessment of the client’s strengths and needs, identification of objectives, action steps, needed services, and crisis planning.
- Reassembles the Wraparound Team when subsequent progress reviews and revisions are needed, at minimum every 90 days and yearly for the annual progress review. Will contact the client’s family and / or Wraparound team members at least monthly to ensure person centered service plan addresses the client’s ongoing needs.
- Builds collaboration and ongoing coordination among the family, caretakers, service providers, and other formal and informal community resources identified by the family. Promotes flexibility to ensure appropriate and effective service delivery to the client and their family.
- Ensures all SED waiver guidelines and rules are followed (Medicaid compliance).
- Completes clinical documentation (treatment plans, progress notes, utilization reviews, etc.) in a timely fashion. Stays informed of agency developments by reading memorandums and other agency related written communications.
- Respect and demonstrate ability to maintain client confidentiality.
- Act as an ambassador of Family Service and Guidance Center in the community setting. This may include doing public educational and informational presentations.
- Supports and actively promotes the vision and mission of Family Service and Guidance Center and the philosophy of the strengths perspective.
- Helping families navigate the Kansas Medicaid System.
- Frequent collaboration with Managed Care Organizations to ensure coverage for client services.
- Other duties as assigned by respective team manager or supervisors.

Other Physical Requirements :
WAF’s must have sufficient ambulatory capabilities to work with children and adolescents in the office and community setting. Position may require the physical holding (MAB) with children and adolescents exhibiting out of control behaviors.
