SUMMARY
Family Service League is seeking a full-time Case Manager for the REAP Treatment program in Mastic, NY. The Case Manager will provide support and case management within a multidisciplinary team to those struggling with substance use and other co-occurring disorders via street outreach and referrals from internal and external sources.

RESPONSIBILITIES
- The Case Manager will be a part of a team conducting community-based work that serves all of Suffolk County focusing on substance use disorders but not limited to the full range of DSM diagnoses, including mental health.
- Work as part of a multidisciplinary team, partnering with Social Workers, Peers & CASACs to work collaboratively on clients needing assessments and linkage.
- Coordinate & collaborate with FSL programs and community providers to link clients to community resources and other clinical providers.
- Engage with outside providers to complete community/homeless outreach to provide harm reduction education and supplies, linkage to services and support.
- Provide outreach to clients in order to facilitate their engagement in services and offer additional supports so they can engage in treatment and live successfully in the community.
- The Case Manager will provide crisis de-escalation and linkage to aftercare services.
- Provide concrete service delivery to clients i.e. benefits/enrollments as per individual needs.
- Provide telephonic as well as face to face outreach, engagement and service planning.
- The Case Manager will provide opioid overdose education and train individuals and families to use Narcan.
- Coordinate care planning with other providers of services/ resources to ensure goal directed, collaborative care, including care transitions.
- Act as a resource to all team members on psychosocial and/or substance abuse issues and resources.
- Attend events in representation of the agency (resources fairs, etc.)
- Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
- Act as a linkage to community services, including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.
- Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.
- Abide by FSL’s Code of Ethics.
- Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.
- Set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information and activities.
- All other duties as assigned.
QUALIFICATIONS
Bachelor’s degree in a behavioral health field required. Master’s degree preferred.
At least 1 year of related experience as a Case Manager in a behavioral health environment required.
Demonstrated experience in the interdisciplinary planning process and the developmental treatment model required.
Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System required.
Knowledge of Medicaid, Social Security and other entitlements preferred.
Proficient computer skills, including Microsoft Office and Electronic Health Records required.
Excellent interpersonal and verbal and written communication skills required.
Strong organization and multi-tasking skills required.
Valid and clean NYS Driver's License required.
Bilingual in Spanish preferred.
