About the Job:
We are seeking a highly motivated Peer Specialist to join our team at Family Service League Inc. The successful candidate will be responsible for providing outreach support and coordination of care for clients in our Certified Community Behavioral Health Clinic (CCBHC) in Riverhead.
About the Role:
This is an exciting opportunity to join our team as a Peer Specialist. As a key member of our multidisciplinary team, you will play a vital role in providing outreach support and coordination of care for clients.
Key Responsibilities:
- Develop and implement individualized plans to support clients in achieving their goals.
- Provide coaching and recovery support to clients, focusing on self-advocacy, linkage to aftercare, wellness self-management, and more.
- Work collaboratively with healthcare professionals to coordinate care planning and provide expert input on client needs.
- Maintain accurate records and statistics, ensuring timely and organized reporting.
Requirements:
- A High School Diploma or equivalent is required.
- Prior experience as a peer specialist in a related setting is preferred.
- The ability to draw upon personal experience living with a disability and/or social/emotional challenge is essential.
- Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System is an asset.
- Excellent interpersonal and verbal and written communication skills are required.
- Proficient computer skills, including Microsoft Office and Electronic Health Records, are necessary.
