The Position:
We are seeking a skilled Case Manager to join our Home Base program in Bay Shore. As a key member of our team, you will provide direct service to our target population, working closely with OMH, DSS, and FSL parameters. Travel is required, and you will participate in agency-wide initiatives and collaborative projects outside of the organization.
Key Responsibilities:
- Provide case management services to youth in the program
- Conduct home visits to assess client needs and develop plans
- Collaborate with skill builders to achieve client goals
- Document all client interactions and track program milestones
- Participate in an interdisciplinary team to ensure comprehensive care
