Social Worker Job Description:
We are looking for a compassionate and skilled social worker to coordinate and implement effective care plans for our residents. Key responsibilities include:
  • Maintaining and improving the residents' ability to manage their physical and mental needs.
  • Identifying and addressing individual social and emotional needs through tailored care plans.
  • Developing and implementing comprehensive care plans in collaboration with interdisciplinary teams.
  • Participating in admission planning, community re-entry, and transfer processes.
  • Ensuring timely treatment of social and emotional needs through regular care plan reviews and updates.
  • Collaborating with finance teams to educate residents about their financial situation and ensure proper documentation.
  • Coordinating discharge planning, including arranging necessary DME or home care services for a seamless transition back into the community.

Qualifications/Requirements:
To be successful as a Lead Social Services Coordinator, you should possess the following qualifications:
  • Bachelor's degree in Social Work (BSW) or Master's degree in Social Work (MSW) preferred.
  • At least 3 years of experience in social work required.
  • Ability to work flexible schedules, as needed.
  • Prior experience in post-acute care settings is highly desirable.

What We Offer:
We offer a competitive compensation package, including up to $70,000 per year, and a supportive work environment that values employee growth and development.
