Schedule
The Case Manager will be responsible for direct service delivery to target populations, completing program documentation as needed, and tracking program milestones and outcomes.

Responsibilities
- The Case Manager will conduct linkage, coordination, advocacy, mentoring, and skill-building services.
- Provide anger management and parenting skills to youth and their families.
- Foster goal achievement surrounding factors contributing to mental health services involvement.
- Provide case management services and skill-building services to youth in the program.
- Conduct home visits throughout Suffolk County.
Qualifications
Bachelor's degree in social work or related field preferred. At least four years of experience working with families in a related capacity, or a Bachelor's Degree with two years of experience.
Excellent interpersonal and verbal communication skills required.
