Key Responsibilities:
- Develop Individualized Planning in areas such as health and education
- Provide linkages and support to achieve client goals
- Collaborate with mental health providers on crisis planning
Requirements:
- Bachelor's degree in Social Work or related field
- 2 years of relevant experience
- Valid NYS Driver's License
