What are the responsibilities and job description for the Service Coordinator position at Family Services Association?
- Provide case management services
- Develop and lead groups for the participant.
- Assist the program coordinator in referral, admission and discharge process.
- Write progress notes quarterly.
- Inform participants and their families/caregivers of available community services, and make referrals as indications warrant.
- Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed).
- Assist, as necessary, in the delivery of other required program services.
- Participate in the bi-annual or quarterly review of each participant’s care plan as well as assisting with the development of care plans.
- Assist Program Coordinator with the implementation of policies and procedures.
- Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure.
- Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator.
- Education and Experience Requirements:
- Bachelors Degree in Health or Human Services or a related field required.
- At least One (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required.
- Significant work experience with the elderly and/or physically/mentally challenged adults in a community setting preferred.
**Must be able to successfully complete a MA State CORI check, CPS check and company paid, and pre-employment physical.**
