Description of Agency:

Family Services of Davidson County is a non-profit agency located in Lexington, NC providing counseling, crisis intervention, and youth development programs to strengthen individuals, families, and our community. We work from a team model and Trauma-Informed Care approach to provide optimal services to our clients and to promote a healthy workplace environment.

Primary Function:

This Social Worker position will be responsible for providing in-home counseling and case management using the Family Empowerment Program model. The Social Worker will work with families who are at risk of involvement with the Department of Public Safety, Division of Adult Corrections, and Juvenile Justice.

Job Type: Full-time salaried position

Education/Experience:

1. A BSW with case management experience OR a human services degree with experience in social work case management and/or program management.

2. Demonstrated competence in case management and/or program management with a solid theoretical base in the helping process that includes the Family Systems approach.

3. Demonstrated administrative competence in documentation.

4. Demonstrated ability to function as a consultant and advocate in the community.

Accountability:

Works under the direct supervision of the FEP Leader and Operations Director

Responsibilities:

· Perform Social Work assessments including all required paperwork such as consent forms, release forms, and eco-maps.

· Provide in-home services to Davidson County families to encourage utilization and follow-up services.

· Develop and maintain individual service plans for each client family according to appropriate program guidelines.

· Maintain confidentiality of all information regarding client family.

· Review social work plans and follow-up with the client, his family or legal custodian, staff members, school, and any agencies involved in the case.

· Communicate regularly and attentively with referral sources including DPS, schools, DSS, etc.

· Attend team meetings for the purpose of participation in coordination and evaluation of services to clients.

· Provide social work services that may include coordination of transportation, group activities, school visits/advocacy, and counseling services.

· Keep the Youth and Community Services Program Manager informed of the Family Empowerment Program’s effectiveness, needs and problems.

· Develop and maintain a current resource file for this population.

· Secure data as applicable for reporting proposes.

· Attend juvenile and truancy court as a representative of the agency to advocate for clients and FSDC’s programs.

General:

· Promote the Family Services philosophy to the community.

· Process all correspondence, time and travel sheets, and monthly reports as scheduled.

· Represent agency at meetings when requested and participate in fundraising events.

· Carry out any additional assignments as requested by the supervisor or director.
