Position Title: Homecare Services Provider Agency (HCSPA) Program Manager
Reports To: Executive Director
Job Overview
The Program Manager is responsible for the daily management of Homecare services provided by the agency this include but not limited to coordinating services, overseeing caregiver recruitment and support, managing relationships with care teams, ensuring compliance with Medicaid and other healthcare regulations, and developing strategies to improve the program’s effectiveness.
Key Responsibilities
Program Management:
- Oversee the development, implementation, and management of our Homecare Services.
- Manage program goals and outcomes, ensuring adherence to the mission of providing family caregivers with the support and resources needed to care for individuals in the home and community setting.
- Ensure that services are delivered in compliance with federal and state regulations, including Medicaid guidelines.
Caregiver Support:
- Provide leadership in recruitment, onboarding, and retention of family caregivers.
- Offer resources, training, and support to caregivers to help them succeed in their roles.
- Work directly with caregivers to troubleshoot challenges and offer guidance on best practices for caregiving.
- Monitor caregiver satisfaction and address concerns in a timely manner.
Compliance and Quality Assurance:
- Monitor and evaluate the program's performance, ensuring that all activities comply with state and federal regulations, including health and safety standards.
- Conduct regular audits to ensure the quality of care and adherence to Medicaid guidelines.
- Provide reporting to state agencies and senior leadership on program outcomes, challenges, and opportunities for improvement.
Collaboration and Communication:
- Serve as the primary point of contact for the Structured Family Caregiving program, fostering relationships with family caregivers, healthcare providers, and other stakeholders.
- Collaborate with the Missouri Department of Social Services and Division of Developmental Disability and other health services agencies to enhance program delivery and identify funding opportunities.
- Advocate for the needs of caregivers within the state healthcare system, identifying policy or procedural changes that could improve the program.
Training and Education:
- Develop and implement caregiver training programs focused on enhancing skills related to daily caregiving, medical management, and safety.
- Coordinate ongoing education and development opportunities for caregivers, including workshops, online courses, and peer-to-peer support networks.
Budget and Resource Management:
- Develop and manage the budget for the program, ensuring that resources are allocated efficiently and effectively.
- Identify funding opportunities and assist in securing grants or other resources to support program initiatives.
Data Collection and Reporting:
- Maintain accurate records related to caregivers, clients, and service outcomes.
- Generate and analyze data on program performance, caregiver satisfaction, and patient health outcomes.
- Prepare and present regular reports to state agencies, senior leadership, and stakeholders.
Skills & Abilities:
- Strong organizational skills and the ability to manage multiple priorities simultaneously.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to analyze data, prepare reports, and make data-driven decisions.
- Strong problem-solving skills, with the ability to develop innovative solutions to challenges faced by caregivers and program participants.
- In-depth understanding of healthcare regulations, Medicaid, and aging services.
Other Requirements:
- Knowledge of the Missouri Department of Social Services and Division of Developmental Disability services is a plus.
- Ability to work independently and as part of a team.
- Travel may be required to meet with caregivers, healthcare providers, and other stakeholders across the state.
Work Conditions
- Hours: Full-time, standard business hours, with flexibility as needed to accommodate caregiver schedules or urgent issues.
- Location: Primarily office-based with potential for some remote work, occasional travel required.
