Substance Abuse Case Manager Job Description:
We are seeking a compassionate and dedicated Substance Abuse Case Manager to join our team. In this role, you will provide support and advocacy for individuals struggling with substance use disorders, helping them access treatment, resources, and recovery services. You will work closely with clients, healthcare providers, and community organizations to develop personalized treatment plans that promote long-term sobriety and overall well-being.
Must provide own transportation to provide home visits.
SKILL SETS: Case Management for FCC must have the ability to provide adequate communication skills, process verbal and written instructions, be dependable, interact effectively with the public and staff, adaptability with being able to deal with frequent change, delays or unexpected events, and a strong sense of commitment. Case Management requires excellent communication, interviewing, documentation and organizational skills as well as the proven ability to efficiently use a computer and communication systems.
PHYSICAL AND ENVIRONMENTAL DEMANDS: Visual acuity necessary for updating files and completing documents. Requires frequent kneeling and crouching to file records. Hearing and talking is necessary to answer phone and talk with employees and other individuals. Work is light with walking, standing and sitting. Manual dexterity is required to perform work processing and other clerical duties. Possible exposure to bloodborne pathogens, biohazardous waste, hazardous waste and communicable diseases.
Key Responsibilities:
- Conduct assessments to determine clients’ needs, strengths, and challenges.
- Develop and implement individualized treatment and recovery plans.
- Provide case management services, including referrals to medical care, counseling, housing, and employment support.
- Monitor client progress, adjust care plans as needed, and provide crisis intervention when necessary.
- Advocate for clients and coordinate services with community partners, treatment facilities, and social service agencies.
- Maintain accurate and confidential client records in compliance with legal and organizational policies.
- Educate clients and their families about substance use, treatment options, and relapse prevention strategies.
- Participate in team meetings, training sessions, and professional development opportunities.
Qualifications:
- Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s preferred).
- Experience in substance abuse treatment, case management, or social services.
- Knowledge of addiction recovery models, harm reduction strategies, and community resources.
- Strong communication, problem-solving, and organizational skills.
- Ability to work effectively with diverse populations and individuals in crisis.
- Certification in substance abuse counseling (e.g., CADC, LADC) is a plus.
