Reports to : TCM Coordinator & Clinical Supervisor
Role- What you do
- This position is responsible for the case management of mental health / substance use 25 individuals; for creating and insuring implementation of individualized care plans to address significant areas.
- Link individuals and families to needed resources, coordinate services, monitor progress and services, and facilitating meetings and monthly visits.
- Ensures that each individual receives services appropriate to the youth's individual care plan.
- Enters case notes in a timely manner, documenting all interactions with youth, family, involved agencies, schools, etc., concisely summarizing those interactions.
- Collaborates with the therapist and other service providers to develop a individualized care plan with the involvement of the youth's family and any involved agencies.
- Communicates regularly with treatment team members and family, as appropriate.
- Assists with meeting the needs by linking them with the needed resources.
- Attends all treatment team meetings and is prepared to offer suggestions concerning treatment strategies.
- Participates in on-call schedule.
- Ensures that appropriate discharge planning is in place
- Ensures that state, placing agency, and RE documentation and all paperwork are completed in a timely manner.
- Participate in networking activities (e.g. network with community stakeholders, public speaking, and other pertinent activities), and in community workgroups.
- Perform other duties as assigned.
- Assists in crisis intervention efforts to maintain the safety of the clients.
- Coordinates discharge planning as an ongoing process of treatment. Provides referrals to alternative aftercare prior to discharge of clients.
- Keeps Clinical Supervisor apprised of all significant treatment issues within the program.
- Maintains on-call responsibilities for crisis intervention and clinical direction as needed.
