Position Summary
The Support Coordinator is primarily responsible for coordinating support services to persons with intellectual disabilities and/or Autism and their families/caregivers. Major functions involve locating, coordinating and monitoring supports for individuals. There are 3 levels of Supports Coordinator: SC1, SC2 and Lead SC as well as Casual. Full time Supports Coordinator's will have the opportunity to advance to an SC2 or Lead SC based on education, experience, and performance.

Assignments
General Assignments
- This is not a remote position. Employee can expect to spend at least 60% of their time working in the field and in-person inside the homes of older adults and the remainder at Vintage Center for Active Adults
- Must provide their own transportation.
- In-person interactions and conversations are core to The Wise Neighbor Project's success. This position is based at the Vintage location. Extensive local travel will be required within a 5-mile radius of Vintage. At times it may be necessary to travel to other Familylinks locations for training and meetings.
- Lifting up to 10 lbs. maximum and occasionally carrying such articles as files, ledgers and small tools.
- A large amount of walking and standing may be necessary in carrying out duties.
- This position is grant-funded, and therefore, it is limited in time to the duration of the grant.
- This position is eligible for health, dental, and vision benefits as well as 403b retirement plan, life insurance and paid time off.
- This position allows for a semi-flexible schedule due the need for meeting with identified individuals around their schedule.
Education & Qualification
- Bachelor's degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science.
- Two years' experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service.
OR
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.
OR
- A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years' professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.
- A valid driver's license along with access to a vehicle.
- Proficiency with MS Office, computer/laptop, tablet and cell phone required.

- Coordinate consumer appointments to maximize efficiency while prioritizing multiple functions and tasks.
- Ability to work independently.
- Awareness that cultural differences and similarities between people exist without assigning them a value.
- Maintain confidential information within HIPAA guidelines and organizational policies.
Working Conditions
- Flexible scheduling may be required.
- Must be able to travel to the various office and community locations.
- Work will be completed remotely at home, within the office, or other community location maintaining HIPAA compliance.
- Ability to lift and carry light boxes of files and supplies (less than 10 pounds) on occasion.
- A limited amount of walking and standing may be necessary in carrying out duties.
Required Training
- Complete the SC orientation training upon hire as well as 24 hours of training the first year.
- Annually, 24 hours of required, applicable training per year
