Reports to: Mental Health Care Coordinator
Hours: Part time, hourly rate. About 24 hours per week which would include one evening shift on Thursdays, as agreed.
Spanish language skills required.
Description:
Assist in managing mental health clinic, ensuring efficient patient flow and utilization of resources are maximized. Work directly with patients to access needed mental health care supports and services. Equal opportunity employer.
Qualifications:
High School Diploma. Certified Medical Assistant or experience/education in human services
or equivalent a plus
· Must be friendly, compassionate, approachable; must be well organized, detail oriented and efficient; must have a positive attitude and be a team player; must be diplomatic and able to see all sides of an issue.
· Possess excellent written and verbal communication skills
Duties and Responsibilities:
· Initiate patient encounter by entering vital signs and screening responses. Assist patients logging onto telehealth counseling or psychiatry appointments.
· Ensure online caregivers without access to health record have current patient information, especially medication history, vital signs, and current problems.
· Maintain communication with all members of the treatment team.
· Enter results of point of service tests: urine drug screen, urine hCG, and others within the scope of clinic protocol and prescribed by medical providers.
· Perform pill counts for patients with controlled substance contracts.
· Deliver referrals, lab orders, imaging orders and instructions to patients as prescribed by providers.
· Communicate with Medication Center or pharmacy to ensure Rx’s get filled as prescribed.
· Schedule follow up visits as needed, especially for mental health patients, reconciling schedules.
· Monitor inboxes responding appropriately to needs there by forwarding to appropriate provider, delegating to nursing or front desk volunteers, or contacting patients.
· Maintains patient record by documenting all procedures, verbal orders, and patient communication.
· Ensure compliance with all infection control, safety, and confidentiality guidelines (OSHA, HIPAA, etc.).
· Keep the Patient Care Coordinator and Mental Health Coordinator informed of any volunteer, patient or operational problems that may arise.
· Participate in weekly staff meetings.
· Initiate and follow up on patient community resource referrals for identified Social Determinants of Health needs.
