As we expand our Housing Stabilization Services, we are looking for a dedicated Housing Coordinator to join our team and make a lasting impact.
About the Role:
As a Housing Coordinator, you will provide Housing Stabilization Services to individuals with disabilities, mental health challenges, or chemical dependency who are transitioning from homelessness or treatment facilities. Your primary focus will be to help participants secure and maintain affordable, safe housing while addressing barriers to long-term stability. You’ll work collaboratively with participants to develop personalized strategies for success, build life skills, and connect them with vital resources.
Your Key Responsibilities:
*Deliver Housing Transition Services and Housing Sustaining Services in alignment with person-centered housing plans.
*Maintain a caseload of 10-15 participants, providing 2-3 hours of one-on-one support weekly in their homes or community settings.
*Help participants navigate housing barriers, apply for benefits, and secure housing options.
*Assist with lease negotiation, understanding lease agreements, and ensuring readiness for move-in.
*Support housing stability through early intervention, coaching on tenant responsibilities, and connecting participants to community resources.
*Advocate for clients, assist in crisis situations, and provide transportation as needed.
*Maintain accurate documentation
What We’re Looking For:
We are seeking a compassionate and driven individual with the following qualifications and skills:
*Education: High school diploma or equivalent required; Bachelor’s degree in Social Work, Psychology, Human Services, or related fields preferred.
*Experience: At least one year of experience providing similar services in a community setting or case management role.
*License/Certification: Valid Minnesota driver’s license, reliable transportation, and valid insurance required.
Knowledge & Skills:
*Understanding of housing systems, homelessness, and related challenges, including mental health, chemical dependency, and developmental disabilities.
*Strong critical thinking and problem-solving skills with the ability to make independent decisions.
*Excellent communication (written and verbal), organizational, and documentation skills.
*Ability to work collaboratively within a team and independently with minimal supervision.
*Commitment to providing culturally responsive and person-centered services.
Physical & Mental Demands:
*Ability to work in various settings, including office environments, clients’ homes, and the community.
*Capacity to manage deadlines, interruptions, and a diverse caseload.
Other Requirements:
*Must pass a criminal history background check.
*Willingness to travel up to 50% of the time within the service area.
*Strict adherence to confidentiality and data privacy standards.
Schedule:
Monday to Friday, 8-hour shifts (9:00 AM – 5:00 PM). No weekends required.
