Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients in a hospice setting, ensuring comfort and dignity during their end-of-life journey. This role requires strong clinical skills, empathy, and the ability to work collaboratively with interdisciplinary teams to support patients and their families.
Duties
- Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs.
- Develop and implement individualized care plans based on patient assessments.
- Administer medications and manage symptom control effectively.
- Perform medical procedures such as phlebotomy as needed.
- Maintain accurate medical records and documentation in compliance with standards.
- Provide health coaching and education to patients and families regarding end-of-life care options.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure holistic care.
- Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans accordingly.
- Adhere to all safety protocols and infection control standards.
Skills
- Proficient in MDS (Minimum Data Set) assessments and documentation.
- Knowledge of emergency room (ER) protocols and outpatient care practices.
- Familiarity with Athenahealth software for medical records management.
- Strong understanding of physiology to assess patient conditions accurately.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to provide compassionate support while maintaining professional boundaries.
- Strong organizational skills to manage multiple patient cases efficiently.
