Duties
- Conduct thorough assessments of patients' physical and emotional needs.
- Administer medications and treatments as prescribed, while monitoring patient responses.
- Provide education and support to patients and their families regarding end-of-life care options.
- Document patient care accurately in electronic health records such as eClinicalWorks or Epic.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Manage urgent care situations effectively, ensuring timely interventions.
- Maintain up-to-date knowledge of infectious disease care protocols and memory care strategies.
- Assist in coding for insurance purposes as needed.
- Travel to various patient locations as required, providing in-home care when necessary.
Qualifications
- Registered Nurse (RN) license in good standing.
- Experience in acute care, urgent care, or travel nursing preferred.
- Familiarity with electronic health record systems such as eClinicalWorks or Epic is a plus.
- Strong knowledge of medical terminology and vital signs assessment.
- Previous experience in hospice or palliative care is highly desirable.
- Ability to work independently while demonstrating excellent communication skills.
