Job Purpose
The Healthcare Home Nurse Care Manager will assist the CMHC and/or CPRC with the implementation and integration in daily clinical operations of Healthcare Home and its computer-based disease management programs.
Qualifications
- Possession of a High School Diploma or equivalent;
- Registered Nurse or Licensed Practical Nurse with current Missouri license.
- Five years primary care experience in Nursing and experience with healthcare technology preferred.
- Meet the minimum requirements to be clinically privileged by FCC Behavioral Health.
Core Job Duties
- Assists HCH Director to develop and enhance wellness initiatives, health education groups, activities for consumers, staff and community.
- Monitor critical chronic disease indicators by utilizing Population Health reports in Care Manager for assigned caseload.
- Identify high risk consumers through annual health screening and assessment and make referrals if needed.
- Completion and tracking of all required assessments and screenings for each consumer on caseload.
- Maintain an accurate and up to date consumer record as outlined in the agency and site Policies and Procedure Manual.
- Adherence to all applicable evidence-based practice models.
- Orally communicate information effectively and accurately.
- Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
- Abide by program and agency policies and procedures
- Other job duties and special projects as assigned.
