Salary : $65,000 per year Successful candidates will Be Required to provide documentation during the On boarding process.The Social Worker will work with the program participants, ages 18-24, providing support and counseling to improve the youth and young adult life The Social Worker will alternate between Manhattan and Bronx sites as coordinated by and / or with their supervisor.Essential Job Functions : Provide one-to-one counseling as needed as well as group counseling sessionsMaintain regular contact with participants to meet program standards and achieve program retentionEnsure that referrals are made to appropriate internal and external services as required to meet the needs of the participants and their familiesWrite progress notes on sessions with program participantsMeet with program participants in groups and one-on-oneConduct a thorough assessment, develop a psycho-social that informs the Individual Service Strategy (ISS), and assist with the development of educational and career-related goals collaboratively with participants based on the motivational interviewing processMaintain open communication with program participants and program graduates through face-to-face contact, phone contact, and social media such as FacebookCommunicate regularly with partners who refer and / or supervise participants.Provide outcomes reportingMaintain participant files, including progress notes and reports on progress toward goalsProvide follow-up services for program graduatesProvide weekly reports to the program directorParticipate in regularly scheduled supervision and ongoing trainingAttend meetings and participate in professional development training offeredReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Education and Experience : Licensed Master's Degree in Social Work (LMSW or LCSW (Licensed Clinical Social Worker).Two (2) years of experience providing social work / mental health services to youth and young adults, including one year of experience with disconnecting youth and young adults within the last five years.Experience working within a youth development model is a plusAbility to evaluate and intervene in crisesBilingual in Spanish is a plus.Equal Opportunity Employer
