What are the responsibilities and job description for the Care Coordinator - Health Home Plus (HH+) position at Federation of Organizations?
RiseWell Community Services is seeking a Full-Time Care Coordinator to be a part of the Care Coordination/Health Home plus Program in NYC. This role is remote but requires in-person visits within New York City to ensure meaningful, face-to-face interactions with our clients.
In this position, you will be providing care coordination and health promotion to individuals throughout NYC. Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Assist individuals in obtaining Food Stamps and Social Security benefits. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Manage a specialized caseload of individuals and assist with other duties as needed. Completes all required documentation according to standards and no later than 24 hours of service delivery.

Job Qualifications:
Minimum of a Bachelor’s Degree and 2 years of relevant experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health. Spanish speaking preferred.
