FedPoint is seeking a dedicated and experienced Team Leader, Care Coordinator to join our dynamic Care Coordination team. As a Team Leader, you will be responsible for overseeing a team of Care Coordinators, ensuring high-quality service delivery and exceptional claimant support. This position involves motivating, coaching, and providing guidance to your team, while ensuring consistency in the delivery of services and adherence to policies.
Key Responsibilities:

Team Leadership & Coaching: Supervise, mentor, and coach Care Coordinators to ensure high performance, engagement, and development. Provide feedback, conduct monthly performance evaluations, and assist in the hiring and training of new staff.

Workflow & Escalation Management: Manage care coordination workflows, monitor performance metrics, and handle escalated issues, ensuring prompt resolution of priority concerns and issues from claimants, providers, or legal representatives.

Clinical Risk Management: Collaborate with management to identify and address clinical risks, fraud, and other high-risk issues to ensure compliance and mitigate risk. Participate in high-risk claimant reviews and support the development of strategies for effective risk management.

Process Improvement: Work cross-functionally to improve operational processes and interdepartmental collaboration to enhance efficiency and claimant satisfaction.
Qualifications:

Education & Experience:
-
Current unrestricted RN license in your state of residence (Required)

Minimum of 2 years of supervisory or leadership experience (Required)

2 years of experience in long-term care, with preference given to experience as a FLTCIP (Federal Long Term Care Insurance Plan) Care Coordinator

Ability to uphold compliance and risk management principles and participate in training on an ongoing basis

Location: Portsmouth, NH
Schedule: Hybrid - 2 days in the office, 3 days telework - Monday-Friday
